Accountability Form
Upon settling, scan the Accountability Form. Complete the number of your assigned: Bed, Mattress, Cabinet, Chair and Table.
Repairs and Maintenance
Report immediately any items in your room that needs repair or replacement such as busted lights, hairline cracks, dysfunctional doorknob, and others by approaching the Guard on duty.
Payment Submission Form
Once payment is made, for your payment to be recorded with Centennial 2 Residence Hall, please fill-up the Payment Form IMMEDIATELY If paid through over the counter at Cash Office, ready a picture of your Billing and Official Receipt If paid through GCash Billings, ready a picture/screenshot of your Official Transaction Receipt
CHECK-OUT
This form should be accomplished before your check-out. If check-out will fall on a weekend, resident must submit on Thursday prior the check-out date. Procedure 1. Check your payments with the Admin Office. 2. Request for your copy of Dorm Checklist. 3. Fill out and submit the online Check-out Form.

Errands Form
All residents of Cente 2 must be inside the residence hall by 10 PM. If you need to stay out past curfew, leave before 6 AM, or spend the night elsewhere (e.g. home), please complete this form. Ensure that your errand does not conflict with the INSTRUCTIONS provided by your PARENT/GUARDIAN Fill out this form in advance, Mondays to Thursday, at least before 5 PM of departure date. For weekend errands, kindly submit your errands by 12 noon of the preceding Friday.
Ensuite Toilet and Bath Cleaning Sign-Up Sheet
Through this form, you may request for a specific date and time window during which you authorize staff to enter your room to perform scheduled monthly assessment and cleaning of ensuite toilet and bath.
